The Outer Eastern Area Partnership has brought together a diverse group of professionals and community members to co-design innovative approaches that support young people transitioning from out-of-home-care.

The group are passionate about making a difference for young people in these circumstances and want to support them in accessing sustainable housing, building skills for employment and connecting to their local community.

The co-design process has been adopted to create and test new ideas to identify solutions.

Our co-design teams comprise of young people and carers, representatives from state and local government, community service organisations, businesses and local community members.

Co-design Sprints are facilitated by Peer Academy and sponsored by the Outer Eastern Children and Youth Area Partnership.
